To whom and for what purpose your personal data will be transferred

Your personal data, within the framework of the Basic Law on Health Services No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Organizations, the Regulation on Private Hospitals, the Regulation on the Processing and Protection of Privacy of Personal Health Data, the regulations of the Ministry of Health, Articles 8 and 9 of the POPD No. 6698 and other legislation provisions, for the purposes specified in Article 2 above;

The Ministry of Health, sub-units affiliated to the ministry, the General Directorate of Security and other law enforcement agencies, Judicial authorities (Courts and Execution Offices), laboratories, medical centers, ambulances, medical devices and health service providers in the country with which the company cooperates for medical diagnosis and treatment, the health institution to which the patient himself/herself applies, (Medical centers, hospitals), your legal representatives that you have expressly authorized, lawyers working for the company, regulatory and supervisory institutions and official authorities. No data is transferred abroad.

Methods of Collection of Your Personal Data and Legal Reasons

Provided that your personal data complies with Articles 5 and 6 of the POPD, your personal data is collected in electronic or physical media, fully or partially automatically or non-automatically, provided that it is part of any data recording system. The legal reason for the collection and processing of your personal data detailed in Article 1;

-Ministry of Health regulations, - Turkish Code of Obligations No. 6098, - Turkish Commercial Code No. 6102,

-Turkish Penal Code No. 5237 and Other Legal Legislation Provisions.

Your Rights as Data Owner (Data Subject)

In accordance with Article 11 of the POPD titled "Rights of the Relevant Person" and the additional paragraph dated 07/05/2010 of Article 20 of the Constitution of the Republic of Turkey titled "Privacy of Private Life", you have the following rights if you apply to GÜLSÜM TETİK as a relevant person;

To learn whether your personal data is being processed. To learn the purpose of processing your personal data and whether they are used in accordance with their purpose. To know the third parties to whom your personal data is transferred. To request correction of your personal data in case of incomplete or incorrect processing. To request notification of the transactions made in accordance with paragraphs (d) and (e) of Article 11 of the POPD to third parties to whom your personal data has been transferred.

Your Application to GÜLSÜM TETİK as Data Owner (Relevant Person)

As the personal data owner (Relevant Person), your applications to Gülsüm Tetik will be answered positively or negatively by GÜLSÜM TETİK within 30 days, with justification. Although the applications to be made by the parties are free of charge as a rule, if the transaction requires cost, GÜLSÜM TETİK reserves the right to request the cost.
